Modern NHS Digital Workforce Navigator

Explore the digital, data, architecture, AI, cyber and clinical informatics capabilities a modern NHS organisation needs, how difficult they are to recruit and retain, and when substantive employment is preferable to contract capacity.

Evidence-led workforce guide

Recruitment and retention scores are professional judgements informed by current NHS recruitment examples and UK labour-market benchmarks. Agenda for Change figures use England 2026/27 as a comparator. Actual NHS banding must follow job evaluation, and devolved-nation pay arrangements differ.

Executive judgement

Own the capabilities that preserve judgement and institutional memory.

The strongest model is not “permanent versus contractor”. It distinguishes enduring authority from temporary capacity.

Core position

Enterprise architecture, data architecture, data governance, clinical informatics, terminology, clinical safety and the accountable layer of AI and cyber architecture should normally be retained as substantive capability. These roles make decisions whose consequences outlive individual programmes.

Contractors are most useful for rapid mobilisation, a scarce technology, a migration, an assurance peak or a delivery surge. If a contractor leaves and the NHS can no longer explain why its architecture, models or controls look the way they do, contingent capacity has become capability debt.

Market reality

Architecture, AI, security and specialist data roles face the greatest recruitment pressure. In 2026 NHS England used Recruitment and Retention Premia on several senior architecture and AI roles, evidence that the public-sector pay framework does not always match the external market.

These are examples of labour-market pressure, not universal NHS pay rules.

Own

Enduring authority, architecture intent, safety responsibility, semantics and operational knowledge.

Blend

Substantive accountable leads plus contractors for transition, specialist depth and delivery peaks.

Buy temporarily

Bounded migrations, scarce product expertise and short-term surge capacity with explicit exit and knowledge transfer.

Workforce landscape

Recruitment difficulty × retention risk

Scores are professional judgements informed by NHS and external-market evidence, not vacancy statistics. Labels are shown only for the selected role so the plot remains legible.

Workforce decision aid

Start with the problem and the duration of accountability.

The recommendation separates the role required from the employment model used to acquire it.

Substantive versus contract

Compare cost, continuity and scarcity rather than day rate alone.

The cost tool annualises contractor rates at 220 days and uses an adjustable substantive on-cost assumption.

Use local finance assumptions for a real business case.

Workforce anti-patterns

Capability debt appears after the programme has moved on.

These patterns should be treated as design risks in workforce planning.

Architects by the projectEvery programme hires architects, but nobody owns architecture between programmes.
Contractor transformation logicCritical mappings and pipelines are understood mainly by people whose contracts end at go-live.
AI without MLOpsModels are piloted without durable ownership for deployment, monitoring, evaluation and retirement.
Governance as a documentA framework is delivered without substantive stewardship and decision rights.
Under-banded scarce rolesInternal grading does not reflect the experience required or the external labour market.
Supplier as semantic memoryThe organisation owns the data but another party remains the only one that understands its models and mappings.
